Unlike reporting only the highest Clavien-Dindo grade, CCI integrates ALL post-operative complications weighted by their severity. A patient with multiple minor complications scores higher than a patient with a single moderate one.

CCI Formula

CCI = 100 × [1 − ∏(1 − wᵢ/100)] Where wᵢ = weight of each complication: Grade I = 8.7 Grade IIa = 20.9 Grade IIb = 26.4 Grade IIIa = 37.0 Grade IIIb = 41.3 Grade IVa = 49.7 Grade IVb = 57.2 Grade V = 100 For a single complication: CCI = weight of that complication ∏ = product (multiplication) over all complications

CCI uses a multiplicative non-independence model — each additional complication adds to the burden even if minor. A patient with three Grade IIa complications has a CCI of ~52.5, not 20.9.

CCI vs Highest Clavien-Dindo

Patient A has three Grade IIa complications:
CCI = 100 × [1 − (1 − 0.209)(1 − 0.209)(1 − 0.209)] = 52.5

If only the highest Clavien-Dindo grade (IIa) were reported, the complication burden would appear minor. CCI captures the true morbidity burden from multiple complications.

  • CCI was developed and validated in 8,085 patients from University Hospital Zurich (Slankamenac 2013)
  • CCI is the preferred outcome measure for surgical quality improvement and clinical trials over "any complication" or "major complication" rates
  • CCI correlates with length of stay and 30-day readmission better than Clavien-Dindo alone
